This is the contents of my Letter of Agreement so you can look through what’s included in my contract, in your own time.

If you choose to move forward with my doula services, I will send you a personalised contract through a secure online eSignature portal for you to review and sign.

Birth Doula Service Agreement

1. Purpose of This Agreement

This agreement is designed to create a clear, supportive, and respectful partnership as you prepare

for the birth of your baby. It outlines the services provided and ensures mutual understanding between

both parties.

3. Package Selection

4. Role of the Doula

The Doula provides continuous emotional, physical, and informational support throughout

pregnancy, labour, and birth.

This includes:

◦ Education for informed decision-making

◦ Labour positioning suggestions

◦ Natural comfort techniques (massage, breathing, relaxation)

◦ Emotional reassurance and encouragement

◦ Support for birth partner

◦ Birth environment support

◦ Birth plan guidance

◦ Photo/video support where appropriate

I understand what the role of a doula entails.

5. Scope of Practice

The Doula is a non-medical professional and does not:

◦ Perform clinical procedures

◦ Provide medical advice or diagnosis

◦ Replace your medical care provider

The Doula may support you in understanding options and communicating your preferences but

does not make decisions on your behalf.

I understand what is included in the scope of doula support. I understand that Tanith is not a

medical professional and will not perform clinical tasks, offer medical advice, or replace my care

provider.

6. Childbirth Education

It is important for you to be educated and knowledgeable in the process of your labor and birth to

achieve an empowered and satisfying birth experience. I highly recommend that you and your partner

complete all the modules in Together We Birth/Independent Childbirth education to gain a better

understanding of:

◦ Physiology of birth

◦ Labour expectations

◦ Breathing and relaxation techniques

◦ Mindset preparation

◦ Understanding interventions and associated risks

I understand that I am responsible for my own birth preparation and choices.

7. Availability

◦ On-call period: 37–42 weeks (38–42 weeks for Essentials Package)

◦ Available 24/7 during on-call for labour

◦ Appointments: Monday–Friday, 9:30 AM – 2:30 PM

The Doula requires up to 60 minutes plus travel time to attend your birth.

I understand and agree to Tanith's appointment times and on-call period

8. Backup Doula

There may be times when I am unavailable during your on-call period due to prior commitments,

illness, overlapping births, or unforeseen circumstances. In these situations, a trusted backup doula will

attend in my place to ensure you receive continuous support.

You will be introduced to two backup doulas during your pregnancy and will provided with their

names and contact details, so you feel comfortable and familiar with who may attend your birth if

needed.

If I anticipate any planned unavailability, I will provide advance notice and ensure a smooth handover

of your care.

In the event that you are unable to reach me after two phone calls within 15 minutes, you may

contact one of the backup doulas directly. We work collaboratively to ensure you are always

supported

I understand that Tanith may call on a trusted backup doula and that I will be introduced to both

backups and provided with their full details during pregnancy

9. Client Responsibilities

You agree to:

◦ Inform your care provider of doula support

◦ Communicate updates during labour

◦ Share relevant health information

◦ Engage in birth preparation

I understand that it is my responsibility to disclose relevant information to both my doula and care

providers to ensure appropriate and effective support

10. Communication & On-Call Support

Non-Urgent Communication

For general questions, birth planning discussions, or postpartum resources:

◦ Method: WhatsApp

◦ Hours: Monday to Friday, 9:00 AM – 5:00 PM

◦ Response Time: Within 24 business hours

Non-urgent messages are not monitored outside of business hours to ensure I am well-rested and

fully available for labour support.

On-Call Period

My on-call period begins at 37–42 weeks gestation (or 38–42 weeks for the Essentials Package) and

continues until the birth of your baby.

During this time, I am available 24/7 for labour and urgent concerns only.

If you believe you are in labour, please contact me by phone so we can assess your situation and

determine the appropriate next steps.

Labour Communication Protocol

If you believe labour has started or you have an urgent update, please follow this process:

a. Call me directly

b. If unanswered, leave a voicemail

c. Call again within 15 minutes

Text or WhatsApp messages should only be used after we have made initial contact by phone.

When to Call Immediately

Please contact me at any time (24/7) if:

◦ Your waters have broken

◦ You believe labour has started

◦ You are experiencing regular, intense contractions

◦ You are heading to or have been admitted to hospital

◦ You receive an urgent medical update or change to your birth plan

If you are unable to reach me after two phone calls within 15 minutes, please contact one of my

backup doulas:

We work collaboratively to ensure you are always supported.

I acknowledge that I understand how and when to contact my doula, and that phone

communication is required for urgent or time-sensitive situations.

I understand and agree to follow the labour communication protocol outlined above. I acknowledge

that on-call support begins at 37–42 weeks gestation (or 38–42 weeks where applicable), and that

every reasonable effort will be made to attend if labour begins earlier.

11. Labour Communication & Attendance

Please contact me when you believe you are in active labour. At this stage, I can provide support,

reassurance, and guidance as needed.

It is important to keep me updated on any significant changes or developments, including cramping,

loss of mucous plug, rupture of membranes, or other relevant pregnancy-related changes.

When you require my attendance, you or your partner must contact me by phone. If I attend during

early labour, this may impact my ability to provide continuous support during active labour when you

will need me most.

Please allow up to 60 minutes for my arrival, in addition to travel time.

I will meet you at your chosen place of birth (home, hospital, or birth centre) and remain with you

for the duration of labour and birth unless extraordinary circumstances arise.

You are responsible for informing your care provider in advance that you have engaged my services

as your doula to ensure smooth communication and avoid any misunderstanding upon arrival.

I understand my responsibility to communicate clearly and promptly during labour

12. Changes to Birth Plans & Services

Birth is unpredictable.

If your birth plans change (including induction, caesarean, or other interventions):

◦ Tanith will continue to support you within her scope

◦ Your package may be adapted where appropriate

◦ Full fees still apply unless otherwise agreed

Support may shift (e.g. more prenatal or postpartum focus), but your care remains.

I understand that services may adapt if my birth plans change.

13. Miscarriage or Pregnancy Loss

In the event of miscarriage or pregnancy loss, Tanith will offer full emotional support and

compassionate flexibility with your contract.

You may choose:

◦ A partial refund for any unused support,

◦ Or the option to hold your deposit for future pregnancy support—whichever feels most

comforting and right for you

This does not void the agreement; Tanith will still honour any support already provided and remain

available for debriefing or emotional care if you wish.

I understand that in the case of miscarriage, I can choose a refund for unused support or carry

forward my deposit for future care.

14. Caesarean Birth

In the event of a Caesarean birth, I can continue to provide support in theatre, with permission from

your care provider.

• Hospital policies generally stipulate that only one support person can be present in theatre.

Therefore, if you feel you would benefit from my presence, you will need to request an additional

support person from your care provider at the time.

• If your care provider does not agree, then I will stay with you until you are taken through to

theatre and then pack up your belongings ready to be relocated to your room on the ward.

I understand that Tanith will still support me should a caesarean become necessary

15. After the birth

I will provide immediate support following the birth for up to 2 hours.

• During this time, I will offer any assistance that is required such as encouragement, breastfeeding

support, and/or bonding photos, while also respecting and protecting your right to uninterrupted

bonding time with your new baby.

• If your baby requires admission to the Neonatal Intensive Care Unit (NICU), then I am able to

remain with you, while your partner goes with your baby, so that you are not left unsupported.

I understand that Tanith will provide me with support for 2 hours after the birth

16. Postpartum visit/visits

Within 14 days of your birth, I will schedule a postpartum visit to debrief your birth and answer any

questions or concerns you may have.

• I can provide support and information on baby care, breastfeeding, settling, and helpful tips to

make things easier for you, in addition to referring you to any external support services you may

require.

The Complete Care Package includes an additional three postpartum visits (up to 3 hours each).

These visits are tailored to the Client’s needs at the time and may include, but are not limited to:

• Birth debriefing and emotional processing

• Newborn care education (as requested)

• Support with newborn care to allow rest and recovery for parents

• Gentle breastfeeding support and guidance

• Babywearing education and assistance

• Meal preparation within the home

• Light household tasks (laundry, dishes, general tidying)

• Errand assistance

• Attendance at appointments (as requested)

• Support with establishing family sleep routines

• Referrals to appropriate community support services

I understand that the services offered in the postpartum visit/visits

17. Additional Services & Fees

This section outlines services and fees that fall outside the inclusions of the selected package. All

additional services are subject to availability and must be agreed in advance where applicable.

1. Additional Appointments

Any additional prenatal or postpartum appointments outside of the selected package inclusions shall be

charged at a rate of $200 per two (2) hour session.

Additional time beyond the agreed session duration will be charged proportionally or as otherwise

agreed in writing.

2. Extended Postpartum Support

Where postpartum support sessions are included within a package, any additional postpartum support

requested beyond those inclusions will be charged in accordance with the Additional Appointments fee

structure outlined above, unless otherwise agreed.

3. Birth Pool, Equipment & Hire Items

Hire items (including but not limited to birth pool, birth sling, TENS machine, and rebozo where not

included in the selected package) may be provided subject to availability and will be charged at the

applicable hire rates as advised at the time of booking.

The Client is responsible for the safe use, care, and return of all hired equipment. Replacement or

repair costs may apply in the event of loss or damage.

5. Travel Fees (if applicable)

Where significant travel is required outside the standard service area, additional travel fees may apply.

Any such fees will be discussed and agreed with the Client in advance.

6. Payment Terms for Additional Services

All additional services and fees are payable in accordance with the following terms:

◦ Invoiced upon delivery of service, unless otherwise agreed

◦ Payable within the timeframe specified on the invoice

◦ Must be paid in full prior to or at the time of service delivery where applicable

The Client acknowledges and agrees that any services outside the selected package inclusions are

not included in the package fee and will incur additional charges as outlined in this section

18. Emergency Situations

In the event of an emergency during pregnancy, labour, or postpartum, Tanith's priority is the safety

and wellbeing of you and your baby.

If a situation arises where urgent medical care is required, Tanith will call emergency services (e.g.

ambulance) and transfer care to the appropriate medical professionals.

This may occur even if it differs from your birth preferences or planned approach.

I understand that in an emergency, Tanith will prioritise contacting emergency services if required.

19. Confidentiality

Your personal information will be kept strictly confidential and handled with care.

However, you acknowledge that information may be shared in the following circumstances:

◦ With your consent

◦ If required by law

◦ If there is a concern for your safety or the safety of others

◦ With a backup doula if required for your care.

All photos, stories, or identifiable information will only ever be shared with your written consent.

I understand how my information may be used and protected

20. Refund Policy

The 30% deposit paid upon signing this Agreement is non-refundable.

If the Doula is unable to attend the birth due to unforeseen circumstances (including illness)

and a suitable backup doula is unavailable, a refund of 50% of the total package fee will be issued.

The remaining 50% will be retained for services already provided.

No refunds will be issued where:

◦ The Client fails to provide reasonable notice or adequate time for the Doula to attend

◦ The Client declines the attendance of a backup doula after the Doula has already

provided more than 15 hours of support

◦ The Client elects not to proceed with Doula attendance after engagement

No refunds will be issued in the event of a Caesarean birth, including elective Caesarean.

In the event of an elective Caesarean birth, services will continue in an adapted format as a Positive

Caesarean Doula support service.

The Client acknowledges that birth is unpredictable and that the Doula cannot guarantee any

specific outcome. The Client is responsible for their own preparation, education, and decision-making

throughout pregnancy and birth

21. Payment Information

Payment of all fees must be made by DIRECT DEBIT to:

22. Acceptance of Service Agreement

I agree to pay the non-refundable deposit of 30% of selected package fee for Doula Services

as offered by Tanith Forte. I agree to pay the balance of the fee by Week 36 of my pregnancy.
